Effective July 15, 2020, Governor Bullock's Mask Directive requiring the mandatory use of face coverings for Montanans was put in place and is in effect in all counties with four (4) or more active cases of COVID-19.  Find out more at Governor Bullock's Pressroom.


If you believe a Pondera County business is not following the Governor's most recent directive, please do the following:

  1. Read the Directive to determine if the complaint is valid

  2. Call the State Information Hotline @ 800-755-6672 to verify that the complaint is valid

  3. File an online complaint or use this printable version (fax completed form back to us)

Please submit the following online form to the best of your abilities. Once your form is submitted, a Pondera County Health Department staff member will contact you within 5-7 business days. Please note that this form is only in regards to social distancing requirements laid out by Governor Bullock's Phase 2 Directive. This form will not preclude you from obtaining all other appropriate permits and approvals from other necessary entities in regards to your gathering.
